Application Description:

The MYF mobile app allows users to easily apply for medical care at the MYF Heart Center. Through the app, patients can submit their medical data and upload the required documents to request treatment.

Steps to Apply through the App:

Enter the patient’s full name.

Enter the patient’s National ID number (for adults) or birth certificate number (for children).

Provide a phone number for contact, as well as an alternate number for easier communication.

Upload the necessary medical files to complete the application.

App Features:

Quick and simple application submission process, eliminating the need to visit the center or send documents by mail.

Ability to check the status of your submitted application and track its progress.

Faster response times due to the elimination of physical mail submissions.

Important Note: Applications can be submitted once every six months to ensure efficient processing of requests and provide the best healthcare services.
